[OBJECTIVE] To assess the long-term complications associated with the application of reconstruction plate.
[METHODS] Ninety-seven patients underwent immediate repair by using bendable reconstruction plate for maintainance of space and contour following mandibular segmental resection. Of them, 68 cases were followed up for the final outcomes.
[RESULTS] Skin or mucosa perforation occurred 6 cases, instabilization of screws in 22 cases, and broken of plate in 3 cases. The reasons for skin or mucosa perforation was possibly associated with circulatory disturbance due to postoperative irradiation. The bone absorption and screw loosening occured most frequently in bone defect involving mandibular angle and chin.

악안면 종양 절제술 후 즉각적인 가요성 재건판(reconstruction plate)을 이용한 하악 결손 부위 복원 시 장기적인 합병증 발생 양상과 원인을 규명하는 것.
【방법】
하악 절제술 후 공간 유지와 윤곽 보장을 위해 가요성 재건판을 사용한 97명의 환자를 대상으로 연구가 진행되었으며, 최종 결과 분석을 위해 68명의 환자를 후향적으로 추적 관찰하였다.
【주요 결과】
추적 관찰 결과, 피부 또는 점막 천공이 6례, 나사 고정 불안정이 22례, 재건판 파단이 3례 발생하였다. 특히 하악각과 턱 부위의 골 결손에서 골 흡수와 나사 풀림이 빈번하게 나타났으며, 이는 방사선 치료로 인한 혈류 장애와 불합리한 응력 집중이 주요 원인으로 분석되었다.
【임상적 시사점 (성형외과 의사 관점)】
악안면 수술 후 재건판 고정 시 하악각과 턱 부위는 골 결손으로 인해 응력 집중이 심해져 나사 풀림 및 재건판 파단 위험이 높으므로, 수술 시 고정 부위의 선택과 고정 강도 확보에 각별한 주의가 필요하다. 특히 방사선 치료를 병행한 경우 주변 연부 조직의 혈류 장애로 인해 재건판이 피부나 구강 점막을 천공할 위험이 크므로, 피판의 두께와 피하 조직의 충분한 확보가 필수적이다. 재건판 파단은 피로 한계(fatigue weakness)와 반복 하중에 기인하므로, 장기적인 기능 회복을 고려한 내구성이 우수한 판재의 선택이 중요하다. 성형외과적 접근에서는 재건판 위에 충분한 연부 조직 피판을 덮어 피판의 괴사와 천공을 예방하는 것이 장기적인 합병증 감소에 결정적이다. 방사선 치료 역사가 있는 환자의 경우 조직 치유 능력이 저하되므로, 수술 전 전신 상태 평가와 함께 방사선량에 따른 연부 조직의 내성을 고려한 다학제적 협진이 요구된다.
