Abstract

AI-enhanced robotic hands are rapidly reshaping tumour surgery by merging real-time sensing, precision mechanics, and intelligent decision support, yet current systems still struggle with early lesion detection, limited tactile sensitivity, and inconsistent accuracy across cancer types. This review addresses these gaps by examining how next-generation robotic hands, empowered by multimodal AI, augmented imaging, hybrid guidance, and minimally invasive mechatronics, can improve early tumour localization and safer resections. The study synthesizes insights from urologic, breast, colorectal, gastric, thoracic, and gynecologic oncology to highlight shared trends such as the shift toward personalized robotics, smart biopsy tools, light-mediated theranostics, flexible platforms, and real-time intraoperative analytics. A comparative reading of quantitative and qualitative evidence reveals strong gains in surgical precision and patient outcomes, yet also contradictions regarding cost-effectiveness, reproducibility of AI predictions, and disparity in adoption between high- and low-resource settings. Using a narrative review approach, key findings point to robotic hands with enhanced tactile sensors and AI-driven micro-maneuvering as promising breakthroughs for detecting microtumours, reducing positive margins, and guiding on-table diagnostics. Recommendations emphasize stronger clinical validation, interoperable imaging ecosystems, and ethical design. The implications extend to safer surgeries, shorter recovery, and more equitable cancer care. Limitations include heterogeneous study designs and early-stage prototypes. Future research should explore adaptive learning models, haptic-guided autonomy, and broader trials. Overall, AI-enhanced robotic hands signal a transformative pathway for earlier detection and more precise tumour removal.

Introduction

Introduction
The rapid convergence of artificial intelligence and surgical robotics is redefining cancer care, enabling earlier tumour detection and more precise interventions. Across multiple oncologic fields, AI-enhanced robotic systems equipped with advanced sensing, imaging, and decision-support technologies are transforming intraoperative vision and manipulation. Recent studies in urologic, breast, colorectal, gastric, and gynecologic surgery demonstrate substantial improvements in diagnostic accuracy, surgical precision, and patient outcomes [1, 2, 4, 20, 21]. Increasingly, the literature also highlights real-time multimodal intelligence that allows surgeons to detect tumour margins earlier and execute targeted resections with greater confidence [3, 15, 18]. This momentum suggests an emerging shift toward robotic systems capable not only of handling instruments but also interpreting tissue characteristics directly.
Despite such progress, major gaps persist. Current robotic platforms still rely heavily on surgeon-dependent visual interpretation, making subtle tumour boundaries and micro-lesions difficult to identify. While techniques such as hybrid surgical guidance and intraoperative Raman spectroscopy offer improved tumour confirmation, their use remains confined to specialised centres [9, 15]. Challenges also remain in complex anatomical regions where limited tactile feedback and delayed tissue characterisation restrict precision [5, 11]. Although augmented reality and multimodal AI systems are emerging, their integration into AI-enhanced robotic hands is limited [3, 6]. Moreover, no widely adopted solution enables robotic hands to “sense” malignancy through AI-guided micro-sensors or light-based theranostics [7]. Progress is further fragmented across cancer types, lacking a unified, cross-disciplinary framework [10, 13, 17, 23].
This work advances robotic surgery by integrating established diagnostic and haptic technologies into a unified AI-robotic platform. While force-sensing, haptic feedback, Raman spectroscopy, and fluorescence-guided imaging are individually well-studied [14, 22], their synergistic combination within intelligent robotic hands enables real-time tumor detection and precision removal. The novelty lies not in each component, but in orchestrating these modalities through AI-enhanced control, enhancing surgical accuracy, responsiveness, and early tumor intervention within a single, cohesive system. This study proposes a model for embedding AI-driven sensory mechanisms, micro-imaging probes, and adaptive learning algorithms directly into robotic hands to enhance early tumour detection and surgical precision across diverse cancers [2, 4, 5, 21].

Methods

Search strategy
To capture studies related to AI-enhanced robotic surgery for early tumor detection and removal, a search of PubMed, Scopus, and Web of Science was conducted. Search strings combined terms related to robotics, artificial intelligence, and oncologic surgery, including “robotic-assisted surgery,” “AI-guided surgery,” “tumor resection,” and “minimally invasive oncology.” Boolean operators were applied to refine the search and ensure inclusivity of relevant studies, while filters were set for English-language publications from January 2024 to December 2025, reflecting the latest advances in robotic platforms and AI integration [1, 3, 16]. The search was iterative, with cross-referencing of key articles and review papers to capture emerging technologies and clinical applications not always indexed in standard databases [13, 14].

Eligibility criteria

Studies were included if they reported primary or narrative data on AI-driven robotic surgery platforms, tumor detection methodologies, or clinical outcomes of robot-assisted oncologic procedures across any type of cancer. Both human and preclinical studies demonstrating technical innovation, image-guided interventions, or augmented reality applications were considered [6, 9]. Exclusion criteria encompassed publications lacking methodological transparency, non-oncologic robotic applications, or those focused solely on conventional surgery without AI integration [2, 4].

The convergence of AI algorithms with robotic dexterity enables enhanced localization, margin assessment, and tissue differentiation [1, 2]. Many reviews point to the same outcome, operations are becoming smaller, safer, and more tailored to individual tumour characteristics, with AI-driven robotic hands acting as extensions of the surgeon’s decision-making process [12, 16].
Table 1 summarizes current research on AI-enhanced robotic systems for tumour detection and surgical resection, highlighting both empirical studies and narrative reviews. Columns detail study design, sample size, outcome metrics, type of AI algorithm, validation strategy, and whether the work was conducted in preclinical or clinical settings. Only a few studies provide quantitative performance data, primarily using deep learning models for image or spectroscopy-based tumour classification. Many references are reviews describing advances in robotic-assisted oncology. This table provides a comprehensive overview of the landscape, facilitating comparison of methodologies, AI types, and translational readiness from laboratory prototypes to clinical applications.

Enhanced visualisation and tumour sensing through AI: One of the strongest findings across literature is that real-time imaging enriched with AI analytics significantly improves early tumour detection. In gastric and colorectal oncology, technologies such as augmented imaging, fluorescence guidance, and AI-assisted optical sensing help surgeons “see the unseen” by bringing microscopic tumour margins into view during procedures [4, 18]. Real-time Raman spectroscopy supported by robotic biopsy systems further adds to diagnostic confidence, delivering immediate confirmation of malignant cells [9]. These innovations shift tumour surgery from a predominantly visual skill to one increasingly supported by computational interpretation, offering earlier recognition of subtle lesions.
Robotic dexterity Meets intelligent decision support: AI-enhanced robotic hands excel not only in detection but also in the removal of tumours with ultra-high accuracy. In urologic cancers, systems integrate multimodal AI to optimize needle trajectories, adjust robotic arm movement, and refine cutting paths [3, 13]. Similar advancements are seen in robotic lung cancer surgery, where intelligent robotic hands stabilise movements in complex anatomical spaces [5]. A growing consensus finds that these systems consistently reduce blood loss, shorten operative time, and minimize conversion to open surgery, highlighting their capacity to support clinical goals more effectively than conventional methods.
Hybrid Surgical Guidance and Real-World Clinical Integration: Hybrid surgical guidance where AI combines imaging, navigation, and sensing modalities merges as one of the most transformative features in early tumour management. Studies show that hybrid systems used in robotic urologic oncology allow surgeons to fuse fluorescence, CT overlays, and intraoperative imaging into a unified navigation interface [15]. Real-world results demonstrate improved lymph node detection, clearer mapping of tumour margins, and increased surgeon confidence during challenging resections. Such systems reflect a meaningful shift in clinical workflow: the robot is no longer just responding to commands but actively guiding strategic decisions in real time.
Clinical translation pathway: Developing AI-driven robotic hands for early tumor detection faces strict regulations. Medical devices like these must pass complex approval processes, such as the FDA in the U.S. or CE marking in Europe, to ensure safety and effectiveness [1, 2]. These rules cover not only the hardware but also the AI software, which must reliably perform in real surgical settings. Meeting these requirements can be slow and costly, and any errors can delay clinical adoption. As AI systems learn and adapt, regulators must also ensure continuous compliance, adding another layer of challenge for innovators [3, 14].
Surgeons must adapt to using intelligent robotic hands, which requires new skills and confidence in AI guidance [6, 11]. Traditional surgical training may not fully prepare them for autonomous or semi-autonomous systems. Some professionals may resist changes, preferring manual techniques they trust. Effective adoption needs hands-on training, simulation, and trust-building between humans and machines. Over time, integrating AI into daily practice could improve precision, reduce errors, and support complex procedures, but this shift demands patience and willingness to embrace new workflows [4, 22].
Bringing AI robotic hands to hospitals is expensive, from development to testing and marketing. Healthcare providers need proof that these systems improve outcomes and are cost-effective compared to existing procedures [5, 16]. Insurance reimbursement and pricing models must also be adopted. Without clear clinical and economic benefits, adoption may be slow. Demonstrating consistent tumor detection accuracy and safer surgeries can convince institutions that the investment is worthwhile, balancing innovation with affordability [7, 9].
Personalised Oncology Through Robotics and AI: Another recurring theme is personalization. AI algorithms capable of analysing individual tumour phenotypes, previous imaging, and intraoperative tissue behaviour allow robotic systems to adapt their operational parameters for each patient. This personalization is especially prominent in breast and gynecologic cancers, where tumour location, tissue density, and aesthetic outcomes vary widely among individuals [10, 17, 21]. These findings reveal a broader transformation: robotic oncology is moving from standardised surgical pathways to patient-centric models where every movement of the robotic hand is informed by data unique to the patient.
Theranostics, AR, and Space-Grade robotics: Several studies highlight breakthrough innovations pushing robotic oncology into entirely new territories. Light-mediated minimally invasive theranostics (LMIT) integrate diagnosis and therapy into one robotic workflow, using light-based tools to detect, image, and ablate tumours during a single procedure [7]. Augmented reality platforms embedded in robotic systems offer holographic overlays and depth-enhanced visual markers that improve spatial precision [6]. Surprisingly, robotic systems designed for microgravity, originally targeted for space missions, are now informing ground-based tumour surgery by providing flexible, hyper-articulated robotic hands capable of extreme manoeuvrability [19]. These innovations reflect a broader trend where AI-enhanced robotic hands are becoming adaptable across environments and tumour types.
Sensors, Imaging, and adaptive surgery: AI-powered sensory mechanisms in robotic oncology enable real-time tumour detection through multimodal data integration, enhancing precision during surgery [1, 3, 16]. Advanced tactile, visual, and spectral sensors support intraoperative decision-making [7, 9], while hybrid guidance systems and augmented reality optimize tissue differentiation and minimize collateral damage [6, 15].
Micro-imaging probes provide high-resolution, minimally invasive visualization for early tumour identification and surgical guidance [7, 18, 20]. Integrated with robotic platforms, probes facilitate real-time Raman spectroscopy, fluorescence, and light-mediated diagnostics [9, 19], improving precision resection and patient outcomes in urologic, colorectal, and breast oncology [4, 17].
Adaptive learning algorithms enhance robotic surgery by analyzing surgical patterns, predicting tumour margins, and optimizing instrument movements [11, 14, 23]. These algorithms leverage AI-driven imitation learning, patient-specific data, and multimodal feedback [13, 22], continuously improving precision, personalization, and safety in minimally invasive oncologic procedures [2, 16].
Contradictions and conflicting evidence: Despite consistent enthusiasm, several studies highlight tensions and uncertainties. While many agree that AI improves early detection, some report inconsistent performance when imaging conditions vary or when rare tumour types produce atypical patterns, challenging current AI algorithms [17, 20]. There are also conflicting views on cost-effectiveness. Some reviews cite significant long-term savings due to shorter hospital stays and fewer complications, whereas others argue that high system costs and maintenance pose barriers for low-resource hospitals [8]. Not all tumour locations benefit equally from robotic access; extremely small or deep-seated lesions sometimes require hybrid human–robot manoeuvres that current AI cannot yet fully automate.
Gaps, Limitations, and future needs: A recurring gap across literature is the limited availability of large-scale quantitative evidence. Many studies are based on early trials, narrative reviews, or small case series, leaving a need for multicentre trials with larger sample sizes. There remains insufficient evidence on long-term patient survival outcomes, cost-benefit ratios across healthcare systems, and performance differences between AI models. Another gap is the insufficient integration of diverse datasets needed to train AI models, particularly for rare cancers or unique anatomical variations [23]. Ethical concerns and surgeon acceptance also require further investigation.
Table 2 summarizes recent research on the integration of artificial intelligence (AI) with robotic-assisted surgery across various cancers. It categorizes each study by cancer type or surgical field, highlighting both existing gaps and real-world applications. Gaps include limited clinical validation, early-stage adoption, lack of standardized protocols, and scarce longitudinal or patient-centric outcome data. Real-world applications illustrate how AI and robotics are enhancing precision, improving intraoperative imaging, enabling minimally invasive resections, and supporting complex procedures from urologic and genitourinary cancers to breast, gastric, colorectal, lung, and gynecologic malignancies. Some studies also explore cutting-edge innovations such as autonomous surgery frameworks, augmented reality guidance, and robotic platforms for extreme environments like microgravity. Overall, the table provides a concise overview of where AI-driven robotic surgery is advancing, where evidence is still limited, and the practical ways it is transforming modern oncologic care.

In colorectal, breast, lung, and gastric cancer surgeries, AI-supported robotic platforms refine resection margins, enhance surgeon visibility, and reduce the risk of residual disease through real-time imaging and hybrid guidance [4, 5, 10, 15].
Engineering and integration challenges: Miniaturizing and integrating multi-modal sensors, including tactile, optical, and thermal systems, directly onto robotic end-effectors presents substantial engineering challenges. Ensuring that these sensors do not compromise sterility, dexterity, or form factors is critical for maintaining surgical efficacy [2, 11]. The tight spatial constraints of robotic instruments demand innovative packaging and materials solutions, while preserving robustness against mechanical stresses encountered during complex tumor resections [5, 9]. Achieving seamless sensor integration is essential for real-time tissue characterization and intra-operative decision-making without hindering surgical workflow [1, 12].
Data processing and interpretation: Multi-modal robotic hands generate heterogeneous and high-volume data streams, requiring advanced computational architectures for real-time processing and fusion. Edge computing and AI-driven algorithms are increasingly employed to synthesize tactile, optical, and thermal information, providing intra-operative guidance with minimal latency [3, 14]. Efficient data interpretation is crucial for enabling precise tumor margin detection, reducing reliance on post-operative histopathology, and supporting adaptive surgical strategies [6, 18].
AI robustness and generalization: Embedded AI models must remain reliable across patient-specific anatomical variations, tissue properties, and dynamic surgical conditions. Ensuring robustness involves rigorous model validation, continuous adaptation, and careful mitigation of safety-critical failure modes [13, 16]. Multi-institutional datasets and real-time learning frameworks enhance generalization, enabling AI-assisted robotic systems to deliver consistent performance across diverse oncologic procedures [7, 21].

Recommendations
The literature suggests that future development of AI-enhanced robotic hands should focus on improving multimodal sensory integration combining imaging, tactile feedback, and predictive modelling into a unified system that supports surgeons throughout the entire procedure [12, 16]. Clinical teams are encouraged to adopt hybrid guidance tools and augmented reality overlays to strengthen intraoperative navigation [15, 21].
Further training programmes are also necessary to equip surgeons with the skills to interpret AI-generated insights efficiently. Researchers recommend that hospitals expand validation trials for novel robotic systems, especially in complex tumour presentations such as deep-seated gastric, lung, or gynecologic cancers [5, 18].
To overcome regulatory and implementation barriers in AI-enhanced robotic hand surgery, stakeholders should prioritize standardized clinical validation, rigorous safety protocols, and cross-disciplinary collaboration, while engaging regulatory bodies early to establish clear guidelines. Integrating training programs and pilot studies will facilitate adoption, ensuring patient-centric, ethically compliant, and technologically robust deployment in oncology [1, 3, 16].

Implications
The rise of AI-enhanced robotic hands signals a shift toward earlier and more personalised cancer treatment. For patients, this means better survival prospects and less invasive surgeries that spare healthy tissue and support faster recovery [8, 17]. For healthcare systems, improved accuracy reduces the burden of repeat surgeries and long-term complications, contributing to cost-effective cancer management.
On a broader scale, the integration of AI in robotic oncology strengthens clinical confidence and advances precision medicine standards globally. The implications extend even to future environments, as flexible robotic platforms developed for space medicine indicate potential uses in remote or resource-limited settings [19].

Limitations
AI-driven robotic surgery demonstrates variable accuracy under diverse imaging conditions, including differing illumination, contrast, and tissue morphology, which can reduce tumor detection reliability. Performance also declines for rare or atypical tumor types due to limited training data, resulting in potential misidentification or incomplete resections. These constraints highlight the current dependency on high-quality imaging and comprehensive datasets [1, 3].
The high cost of robotic systems continues to limit accessibility, particularly in low-resource healthcare environments. In addition, real-time imaging technologies such as Raman spectroscopy or light-mediated diagnostics require further refinement before they can be seamlessly integrated into routine clinical workflows.

Future research
Advancing AI robustness requires integrating multimodal imaging, synthetic data augmentation, and transfer learning to address variable visual environments. Expanding curated datasets for rare tumors and incorporating real-time adaptive algorithms could enhance detection accuracy and surgical precision. Combining these strategies may reduce variability, improve generalizability, and ultimately ensure safer, more reliable AI-enhanced robotic tumor interventions [14, 16].
There is also a need for standardised datasets to enhance algorithm training and ensure equitable diagnostic performance. Researchers should explore biomimetic sensory technologies that mimic human tactile perception, allowing robotic hands to identify micro-tumours or early tissue abnormalities that remain invisible in current imaging systems. Further studies on cost-reduction strategies and implementation frameworks would support wider adoption.

Continued research and collaboration will be essential to translate these innovations into routine clinical practice.
