Single-stage functional lateral hemilaryngocricoidectomy for a locally advanced chondrosarcoma invading the hemicricoid.
Abstract
Complete resection of cricoid lesions is often extensive, causing defects that require major reconstruction. We describe the use of a novel reconstruction technique in a patient with open organ preservation surgery to treat a grade II chondrosarcoma centred at the left hemicricoid. The pre-, intra- and postoperative course is described. Circumferential release of the trachea was made, ensuring it was below the right remaining hemicricoid. A stepladder tracheal defect was created and the trachea was advanced superiorly until the left tracheal hemirings were intussuscepted and rotated to recreate the missing left hemicricoid. The patient did not require a tracheostomy, a laryngofissure or a free flap. Our patient underwent several small procedures after the primary surgery and is now stable and symptom-free for 24 months with an excellent functional outcome.
