Comment to: Innovative Surgical Techniques for Gynecomastia: Enhancing Patient Aesthetic Satisfaction.
Abstract
We read with great interest the paper titled "Innovative Surgical Techniques for Gynecomastia: Enhancing Patient Aesthetic Satisfaction" by Hesamirostami, Mohammadhossein et al. (Aesthetic Plast Surg https://doi.org/10.1007/s00266-025-05224-w , 2025). The Authors present a very interesting prospective case series proposing an innovative surgical method "the oval eccentric excision with an inferior pedicle" for the treatment of grade IIb gynecomastia. Gynecomastia represents a very common condition in males, leading to reduced selfesteem, social embarrassment, and psychological distress. Consequently, an increasing demand for surgical correction has been observed in recent years. Since is satisfaction with surgical outcomes represents a common cause of complaints, managing patients' expectations is a key factor in achieving a high level of satisfaction, which remains the primary indicator of treatment success. Thorax profile, quality of the scars and nipple areola complex aspect (NAC) including shape, dimension and position represent the major issues to obtain high level of patients' satisfaction. In many cases, liposuction alone is not sufficient to address gynecomastia due to the presence of extra skin resulting after the surgical procedure and to reduce the risk of the recurrence of the disorder due to the persistence of the gland tissue. Moreover, this technique does not allow for histopathological investigation of the glandular tissue.
