We describe three women with rheumatoid arthritis after augmentation mammoplasty with silicone implants. The possibility for a causal relationship between silicone implantation and arthritis as examples of a "human adjuvant disease" is discussed.

실리콘 유방 보형물 삽입 후 발생한 만성 다발성 관절염 사례 3례를 보고하며, 보형물과 관절염 간의 인과관계 가능성을 인간 부조화 질환(human adjuvant disease)의 관점에서 고찰한다.
【방법】
실리콘 유방 성형술(augmentation mammoplasty)을 받은 여성 환자 3례의 임상 경과를 분석하고, 실리콘 이식과 관절염 발병 간의 인과적 연관성을 문헌 고찰을 통해 논의한다.
【주요 결과】
세 환자 모두 유방 성형술 후 류마티스 관절염(rheumatoid arthritis)과 유사한 만성 다발성 관절염 증상을 보였으며, 실리콘 이식이 면역 반응을 유발하는 부조화 질환의 원인이 될 수 있음을 시사한다.
【임상적 시사점 (성형외과 의사 관점)】
성형외과 의사는 유방 성형술 전 환자 상담 시 실리콘 보형물과 관련된 드문 전신 면역 반응 가능성에 대해 충분히 설명해야 한다. 수술 후 환자가 관절 통증이나 염증 증상을 호소할 경우, 단순한 근골격계 문제를 넘어 보형물 관련 면역 반응이나 전신 질환의 징후일 수 있음을 감별해야 한다. 특히 악안면이나 다른 부위의 이식 수술에서도 실리콘 또는 다른 생체 재료가 전신 면역계에 미치는 영향을 고려한 사후 관리가 필요하다. 보형물 제거술을 고려할 때는 관절염 등 전신 증상이 동반된 경우, 국소적 합병증 예방뿐만 아니라 전신 상태 평가 측면에서 다학제적 접근이 필요할 수 있다.
