A case is reported of late infection in a breast implant, in a 35-year-old female who underwent breast augmentation with a double-lumen silicone prosthesis combined with abdominoplasty. After 8 uneventful postoperative months, untoward and poorly defined symptoms occurred and further exploratory surgery became necessary. Due to a subacute inflammatory process in the entire pocket of the left breast, the implants were removed. Culture demonstrated Klebsiella pneumoniae. After complete healing, a pair of texturized moderate profile implants was placed above the muscle through an areolar access. The clinical history and management of this unusual case are described.
【연구 목적】
유방 성형술 후 장기적인 합병증 중 드문 사례인 후기 감염의 임상적 양상과 관리 전략을 제시하여 성형외과 의사의 예방 및 치료 접근에 기여하는 것을 목적으로 한다.
【방법】
35세 여성 환자가 유방 확대술과 복부성형술(abdominoplasty)을 동시에 시행받은 후 8개월째에 발생한 비특이적 증상을 대상으로 한 사례 보고(case report)이다. 왼쪽 유방 주머니(pocket)의 아급성 염증 과정에서 임플란트 제거술을 시행하고 배양을 통해 병원체를 동정하였으며, 치유 후 재시술을 포함한 임상 경과를 분석하였다.
【주요 결과】
수술 후 8개월이 지난 시점에서 비특이적이고 불분명한 증상이 발생하였으며, 수술적 탐색 결과 왼쪽 유방 주머니 전체에 아급성 염증 과정이 확인되어 임플란트가 제거되었다. 배양 검사 결과 폐렴간균(Klebsiella pneumoniae)이 감염 원인균으로 확인되었다. 완전한 치유 후 유두경계 접근법(areolar access)을 통해 근육 상층에 텍스처된 중간 프로필 임플란트를 재시술하여 성공적으로 관리되었다.
【임상적 시사점 (성형외과 의사 관점)】
유방 확대술(breast augmentation) 후 수개월이 경과한 시점에서도 비특이적인 통증이나 염증을 유발하는 후기 감염이 발생할 수 있음을 인지해야 한다. 특히 복부성형술(abdominoplasty)과 같은 다른 수술과 동시 시행 시 감염 위험이나 미생물 군집의 상호작용에 대한 주의가 필요하다. 폐렴간균(Klebsiella pneumoniae)과 같은 그람음성균이 임플란트 주머니 감염의 원인이 될 수 있으므로, 감염 의심 시 적절한 항생제 감수성 검사 및 배양이 필수적이다. 임플란트 제거 후 재시술 시 접근 경로(approach)와 임플란트 표면 처리(surface texture) 선택이 재감염 예방과 임상 결과에 영향을 미칠 수 있다. 성형외과 의사는 장기적인 환자 추적 관찰 시 비특이적 증상을 간과하지 않고 염증성 질환을 감별 진단해야 한다. 다학제적 협진이 필요한 경우 감염내과의와 협력하여 적절한 항균 요법을 결정하는 것이 중요하다.
