[BACKGROUND] Since the introduction of sentinel node (SN) mapping in breast cancer, extra-axillary lymph node sites of breast tumor drainage are discovered in about one-quarter of cases, especially after intraparenchymal injection. In most such cases, an ipsilateral axillary SN is associated with an extra-axillary SN. Non visualization of ipsilateral axillary SN and extra-axillary SN drainage are often associated with an increased risk of axillary involvement.

유방암에서 센티널 림프절(Sentinel Node, SN) 매핑 시 유방实质 내 주사 후 약 4분의 1의 사례에서 액와 외 림프절로 드레나지가 관찰되며, 비정상적인 드레나지 경로는 액와 전이 위험 증가와 관련이 있음을 배경으로 한다. 본 연구는 양측 유방절제술 병력이 있는 유방암 환자에서 동측 액와 림프절 관여 없이 대측 액와로 센티널 림프절이 드레나지된 희귀 사례를 보고한다.
【방법】
양측 유방절제술(reduction mammoplasty) 병력이 있는 유방암 환자의 림프스캔(lymphoscintigraphy) 영상을 통해 센티널 림프절 드레나지 경로를 추적한 사례 보고(case report) 연구이다. 동측 액와 림프절의 비관여를 확인한 후, 비정상적으로 관찰된 대측 액와 센티널 림프절의 존재를 영상학적 소견을 통해 입증하였다.
