Congenital absence of breast development is a very rare abnormality. It may present as an isolated finding or it may be accompanied by other congenital anomalies. Here we report on a 13.5-year-old girl presented to our pediatric endocrinology clinic because of lack of breast development. She had pubarche since the age of 10 years and was regularly menstruating since the age of 12 years. The patient's medical history was positive for bilateral complete choanal atresia that was diagnosed and corrected soon after birth. Physical examination was unremarkable except for bilateral amazia, that is, absence of palpable breast tissue and hypoplastic areolae, whereas both nipples were formed. Renal ultrasonography and chest radiography were normal. The coexistence of congenital bilateral amazia and bilateral complete choanal atresia suggests that these rare disorders may be related etiologically. The patient is scheduled for breast augmentation.

선천성 유방 발달 부전(先天性乳房發育不全)은 매우 드문 기형으로, 단독으로 나타나거나 다른 선천성 기형과 동반될 수 있다. 본 연구는 선천성 양측 비개폐증(bilateral choanal atresia)을 동반한 선천성 양측 무유방증(congenital bilateral amazia)을 경험한 13.5세 소녀의 증례를 보고하여 두 희귀 질환 간의 병인론적 연관성을 제시한다.
【방법】
10세에 사춘기 음모 발현(pubarche)을 시작하고 12세부터 규칙적인 월경을 시작한 13.5세 소녀의 증례를 분석하였다. 출생 직후 진단 및 교정된 선천성 양측 완전 비개폐증 병력을 확인하였으며, 신체 검사, 신장 초음파, 흉부 방사선 촬영을 통해 유방 조직의 부재와 유두하 발달 저하(hypoplastic areolae)를 확인하고 다른 기형을 배제하였다.
【주요 결과】
신체 검사 결과 유방 조직이 palpable하지 않으며 유두하가 발달 저하된 양측 무유방증(bilateral amazia)이 확인되었으나 유두는 형성되어 있었다. 신장 및 흉부 영상 검사에서 다른 이상 소견은 발견되지 않았으며, 이 증례는 선천성 양측 무유방증과 양측 완전 비개폐증의 공존이 병인론적으로 관련될 가능성을 시사한다.
【임상적 시사점 (성형외과 의사 관점)】
성형외과 의사는 선천성 유방 기형 환자를 진료할 때 비개폐증(choanal atresia)과 같은 상기도 폐쇄 병력이 있는지 반드시 확인해야 한다. 비개폐증 교정 수술 이력이 있는 경우, 전신 마취 전 기도 관리(airway management)가 매우 중요하므로 마취과와의 긴밀한 협진이 필수적이다. 또한 유방 조직의 부재와 유두하 발달 저하는 유방 성형술(breast augmentation)이나 유방 재건술(breast reconstruction)의 술전 계획에 직접적인 영향을 미치므로, 환자 상담 시 기대치 조정이 필요하다. 이러한 선천성 기형의 공존은 발생학적 연관성을 시사하므로, 수술 전 전신 상태 평가 측면에서 다른 장기 기형의 유무를 확인하는 것이 안전하다. 최종적으로 유방 성형술을 계획할 때, 유두의 존재 여부와 주변 조직의 상태를 고려한 맞춤형 수술 기법 선택이 요구된다.
