A New Method of Orbital Septal Fat Flap in Transconjunctival Approach.
Abstract
[BACKGROUND] Orbital fat release is critical in blepharoplasty for correcting tear trough deformities, but traditional transconjunctival approaches often face limitations due to restricted fat volume and pedicle length. This study introduces an innovative technique by reconstructing the orbital fat flap, aiming to improve tear trough coverage and optimize Aesthet outcomes.
[METHODS] The technique is achieved by reconstructing the orbital fat flap. To enhance fat survival, meticulous preservation of the pedicle's vascular supply was ensured during surgery. Microsurgical techniques were employed to precisely dissect and position the fat flap, ensuring full coverage of the tear trough area while maintaining facial symmetry. Postoperative follow-up was conducted to evaluate functional and Aesthet outcomes.
[RESULTS] The modified technique resulted in complete tear trough coverage and significant reduction in postoperative retraction. Patients reported improved Aesthet outcomes, with natural restoration of the lower eyelid contour and preservation of the culturally significant "Lower-lid Pretarsal Roll" appearance. No major complications such as fat atrophy or eyelid retraction were observed.
[CONCLUSION] This new technique for extending the orbital fat flap in blepharoplasty offers better coverage of tear trough deformities, reduces the risk of postoperative complications, and improves patient satisfaction, making it an important advancement in eye contour surgery.
