Lagophthalmos, impairment of closing upper eyelids, can result from various reasons. However, lagophthalmus after a rhinoplasty is both unusual and frustrating. We report a patient experiencing lagophthalmos following rhinoplasty and its management.

비강 성형술(rhinoplasty) 후 발생하는 드물고 치료가 어려운 안검 폐쇄 부전(lagophthalmos)의 발생 사례를 보고하며, 그 임상적 관리 방법을 제시하는 것을 목적으로 한다.
【방법】
2023년 크라니오패셜 수술 저널에 게재된 단일 환자 증례 보고(case report) 연구로, 비강 성형술 후 안검 폐쇄 부전이 발생한 환자의 임상 경과와 치료 과정을 기술한다.
【주요 결과】
비강 성형술 후 안검 폐쇄 부전이 발생하여 환자가 불편감을 호소하였으며, 이에 대한 구체적인 관리 및 치료 과정이 보고되었다.
【임상적 시사점 (성형외과 의사 관점)】
성형외과 의사는 비강 성형술 전 안검 기능과 안와 구조에 대한 철저한 사전 평가를 통해 안검 폐쇄 부전 위험을 예측해야 한다. 수술 중 코의 높이를 과도하게 상승시키거나 연부 조직의 부종이 심해질 경우 안검의 기계적 지지력이 약화되어 안검 폐쇄 부전이 발생할 수 있으므로, 안와 하부 및 안검 지지 구조에 대한 주의 깊은 수술 기법이 필요하다. 안검 폐쇄 부전이 발생하면 각막 노출로 인한 각막병증(keratitis) 및 실명 위험이 있으므로, 즉시 인공눈물, 안연고, 안대 패치 등의 보존적 치료를 시작하여 각막 보호를 우선시해야 한다. 증상이 지속될 경우 안과 전문의와의 다학제 협진을 통해 안검 성형술이나 중재적 치료를 고려할 수 있다. 환자 상담 시 수술의 잠재적 합병증으로 안검 기능 장애가 발생할 수 있음을 사전에 고지하고, 수술 후 안검 폐쇄 상태를 정기적으로 모니터링하는 프로토콜이 필요하다.
