Few reports describe the use of assisted reproductive technology (ART) for fertility preservation and conception in patients with chronic phase chronic myeloid leukemia (CML-CP). We conducted an observational study on fertility preservation and pregnancy outcomes in 18 female CML-CP patients aged 15 to 39 years treated at our hospital from December 2001 to May 2024. The median age at diagnosis was 24.5 years. All of the 10 patients diagnosed after 2013 received information about fertility preservation before treatment initiation. Three of the 7 eligible patients underwent fertility preservation. All patients who underwent fertility preservation started tyrosine kinase inhibitor (TKI) therapy within one month of diagnosis. One patient underwent fertility preservation while in treatment-free remission (TFR) after TKI initiation. Three patients desired pregnancy and achieved 3 live births via natural conception or ART under TFR. No patient lost major molecular response while trying to conceive or during pregnancy and childbirth. Planned pregnancies under TFR allow for both disease control and successful conception and childbirth, with ART being particularly effective.
