Faggot cells and blasts with blebs with intense myeloperoxidase are characteristic of acute promyelocytic leukemia (APL). Whenever there is a suspicion of APL, ATRA should be started, given its potential severity. There are few cases with non-APL with this morphology but are rarely described in acute leukemia with myelodysplasia related changes.
