The determinants for the disproportionate burden of pancreatic cancer in Native Hawaiians are unclear. To elucidate this disparity, we evaluated risk factor profiles and pancreatic cancer incidence in 13,641 Native Hawaiians and 47,240 Whites from the Multiethnic Cohort Study. We observed that Native Hawaiians with diabetes, obesity, and smoking had 3.41-fold, 2.35-fold, and 2.62-fold increased risk, respectively, compared with Whites with no risk factors. Notably, Native Hawaiian men with ≥2 risk factors had >5 times the risk relative to White men with no risk factors. This study emphasizes the need for targeted prevention and screening for this underserved population.
