Pectoralis major myocutaneous flap (PMMF) has become the standard for reconstruction of major defects in head and neck area. Eleven cases, operated over a three year period, in which PMMF was used for reconstruction have been reviewed retrospectively. Nine patients had oral squamous cell carcinoma, one had a basal cell carcinoma of the external ear and one had lost skin and soft tissue of neck following synergistic gangrene. Ten of the eleven flaps survived (success rate 91%). One of the three rib grafts used to reconstruct mandible got infected and had to be removed. Three patients developed wound infections and one had a temporary orocutaneous fistula which closed spontaneously. This brief experience confirms the reliability and efficiency of PMMF for head and neck reconstruction.

두경부 절제술 후 발생한 대형 결손 부위를 재건하기 위해 흉대근 피부근막피판(Pectoralis major myocutaneous flap, PMMF)의 임상적 신뢰성과 효율성을 평가하는 것이 본 연구의 핵심 목표이다.
【방법】
3년 동안 두경부 재건술에 PMMF를 적용한 11례의 증례를 후향적으로 분석하였다. 대상 환자 중 9명은 구강 편평상피세포암(oral squamous cell carcinoma), 1명은 외이 기저세포암(basal cell carcinoma), 1명은 괴저성 괴사 후 목 부위의 피부 및 연부 조직 손상이 있었다. 피판 생존율 및 합병증 발생률을 조사하였다.
【주요 결과】
11례 중 10례의 피판이 성공적으로 생존하여 91%의 생존율을 보였다. 하악골 재건을 위해 사용한 늑연골 이식편 3례 중 1례에서 감염이 발생하여 제거되었으며, 3례에서 상처 감염이 발생했다. 1례에서 일시적인 구피판루(ocutaneous fistula)가 관찰되었으나 자발적으로 치유되었다.
【임상적 시사점 (성형외과 의사 관점)】
PMMF는 혈관 공급이 안정적이고 수술 기법이 비교적 단순하여 두경부 재건술에서 여전히 유효한 표준 재건 방법 중 하나임을 시사한다. 성형외과 의사는 악안면 재건 시 피판의 두께와 색상이 주변 조직과 잘 어울리도록 피판 설계 시 주의해야 하며, 특히 구강 내 재건 시 타액 노출로 인한 감염 위험을 고려해야 한다. 하악골 재건 시 늑연골 이식편의 감염 가능성을 인지하고, 필요시 혈관화된 뼈 피판(bone flap) 등 대안적 재건 전략을 함께 고려할 수 있다. 피판의 부피가 충분하여 공여자 부위(donor site)의 봉합 시 긴장도를 최소화하고, 흉터 형상을 미용적으로 관리하는 것이 중요하다. 또한 노년층 환자나 기저 질환이 있는 경우 피판의 혈류 공급이 원활한지 술 중 지속적으로 모니터링하여 피판 괴사(necrosis)를 예방해야 한다.
