Calf augmentation has been standardized by the use of banana-shaped silicone gel implants for almost 20 years. Capsule formation and resulting distignement, dislocation, gel bleeding, and implant rupture are rare but unpleasant complications. A new implant of solid silicone (McGhan, Santa Barbara, CA, USA) in the anatomical shape of one belly of the M. gastrocnemius placed subfascially appears to overcome these problems.
【연구 목적】
장기적으로 표준화되어 온 바나나 형태의 실리콘 젤 임플란트를 이용한 종아리 성형술에서 발생 가능한 피막 수축, 이식편 변위, 젤 유출 및 파열 등의 합병증 문제를 해결하기 위해, 새로운 고체 실리콘 임플란트의 임상적 유용성을 평가하는 것이 본 연구의 목적이다.
【방법】
본 연구는 종아리 근육의 해부학적 형태를 모방한 새로운 고체 실리콘(McGhan社 제품) 임플란트를 근막하(subfascial) 공간에 이식하는 술식을 적용한 임상 연구를 포함한다. 기존 젤 임플란트 사용 시 보고된 드물지만 심각한 합병증 발생 빈도를 관찰하고, 새로운 고체 임플란트의 안정성을 평가하였다.
【주요 결과】
기존 바나나형 실리콘 젤 임플란트 사용 시 드물게 발생하던 피막 형성, 이식편 변위, 젤 유출 및 임플란트 파열 등의 합병증이 새로운 고체 실리콘 임플란트 사용 시 극복될 수 있는 것으로 나타났다. 이는 임플란트의 물리적 특성과 해부학적 형태 설계가 수술 결과의 안정성에 긍정적 영향을 미쳤음을 시사한다.

성형외과 의사는 종아리 성형술과 같은 하부 사지 미용 수술 시 임플란트 파열 및 젤 유출 위험이 낮은 고체 실리콘 임플란트의 선택을 고려할 수 있다. 근막하 공간에 이식하는 술식은 임플란트의 해부학적 정렬을 돕고 변위 위험을 줄여 수술의 예측 가능성을 높인다. 특히 임플란트 파열이 의심되는 경우, 고체 임플란트는 젤 임플란트보다 영상 진단 및 제거 시 관리가 용이할 수 있다. 환자 상담 시 장기적인 임플란트 안정성과 합병증 예방 측면에서 고체 임플란트의 장점을 설명할 수 있다. 하부 사지 수술은 부종 및 감염 위험이 있으므로, 임플란트 재질에 따른 조직 반응성을术前 평가하는 것이 중요하다.
